Doctors and patients have some of the same interests as Managed Care:

• Patients: Given the experience of the surgeon, the chances of having a C-section, coronary bypass graft surgery, or enlarged prostate gland removed varies without any appreciable difference in benefit.
• Physicians: All around the world, doctors' fees and work is being challenged, while best practices and hard work go unrecognized.

As a Medical Director, I had become expert at reducing length of stay, and occasionally even helping to reduce hospital admissions. Nevertheless, as a doctor, I have found I have little patience when MC's objectives, methods and decisions are insensitive to my patient's needs. In other words, I hate micro-management, but at the same time, I am willing to be constructively reviewed.
